The Intake Officer is responsible for providing a high quality; customer focused centralised intake service to meet the specific needs for clients of the Home and Community Care Program for Younger People (HACC PYP) and Commonwealth Home Support Program (CHSP) referrals.
- This vital client facing role has responsibility for assisting clients to determine the type of supports that compliments their My Aged Care Support Plan.
- The Intake Officer will in discussion with the client prepare Care Plans, Service Agreements and validate Health and Safety access for Support Workers.
- The role has strong cross team responsibilities with service delivery to ensure the client experience is of the highest quality.
The Benefits
This is a temporary part time position until 31 May 2027, working 30.4 hours per fortnight.
The position carries a current starting salary of $73,730 per annum (pro-rata), plus superannuation.
